I’m looking to get my girlfriend playing and would like to have her create her own “master account”. We would be playing from the same internet because we live together. Is this legal or what kind of permission do I need for this? I’d hate to be banned for having two accounts using the same up address.

You will most probably be monitored by GM's with such a setup. If it's really a second person on the second account it's fine.

Me and my two sons play on the same network and sometimes simultaniously but that has never been a problem. But I have another question, since I love playing all different chars and I got all 10 slots used on my account, am I allowed to have another account on the same realm (Mid) if I dont play my two accounts at the same time?

click the "+", account chooser, then click 'new account' when the launcher opens. It will make a second game account tied to the same master account (and is on the same realm)
Under no circumstances are you allowed to have 2 master accounts as one person.
