Yeah i havent seen the champion either for quite some while now...

He should respawn now.

Black Lady spawn rate hasnt changed, but her tether was fixed, so she won't leave the room now.

They will spawn as long as she is aggro. Trouble is she can't reach you, so she's still mad at you, preparing for your return.
