Sometimes, when you try to apply poisons by clicking the icons on the hotbar, the poisons will both go onto the same weapon. Sometimes it works normally, but usually for me, it will just reapply the second poison onto my mainhand again, overwriting the original poison.

It's not really a big deal, but it'd be nice to apply them from the hotbar, so I am wondering if this could be fixed.

Hiya. I've only experienced this when an effect on my off hand does not proc (i.e. killed the mob too quick) during a fight. This means that when I come to reapply, the main hand wep poison is overwritten by the second application.

Working as intended. Poison will overwrite mainhand if offhand is already poisoned.
